JOB DETAILS:

We are seeking an experienced Social Worker, ideally with 2 plus years’ experience working within a hospital setting, working with clients to set up support structures in the community.

• Discharge planning and case management
• Advocating for clients to ensure continuity of care
• Work collaboratively with broader team and community services.
• Provide support to families and carers of patients

SELECTION CRITERIA

• Registered with or eligible for registration with AASW
• Experience in hospital discharge planning and care planning
• The ability to obtain a National Police Clearance and Working with Children Check
