About the Opportunity
An exciting opportunity exists for a Clinical Nurse Consultant (Grade 1) to join the Older Person’s Community Mental Health Service—a multidisciplinary team providing specialist mental health care to older adults in the community. Based in a supportive and person-centred model of care, this role focuses on delivering recovery-oriented, culturally sensitive, and holistic support to consumers in their homes or residential aged care settings. You’ll work collaboratively with families, carers, GPs, and aged care providers to ensure continuity of care and promote mental wellness in later life

What You'll be Doing
The Clinical Nurse Consultant (CNC) is an expert practitioner who, through leadership facilitates evidence based practice. The CNC will promote a service which fosters clinical excellence, research and professional leadership. The CNC contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes. The CNC is responsible for providing services which facilitate effective management of the care in the given specialty.

Where You'll Be Working
SWSLHD Community Mental Health Services provide comprehensive, community-based mental health care across South Western Sydney.

These services cater to different age groups, including children, adolescents, adults, and older persons, ensuring specialised care throughout various life stages. The multidisciplinary teams offer a range of supports, including mental health assessments, crisis intervention, care coordination, and therapy. They work closely with inpatient services, GPs, and other community organisations to provide holistic care.

Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA) 5 years full time equivalent post registration experience Approved post graduate nursing qualifications in a field relevant to the specialty field or such other qualifications or experience deemed appropriate by the organisation.
  2. Describe a time when you provided expert clinical advice and developed a care management plan for an older person with complex mental health needs in a community setting. How did you involve carers and other health professionals in the process, and what was the outcome?
  3. How have you contributed to the development or delivery of education programs or clinical service planning within a mental health specialty? Please include examples of how you incorporated research findings or upheld professional standards in your practice.
  4. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
