The role

Your Life Your Support Coordination is seeking a passionate, empathetic Community Participation & Self Care Superstar to join our team to work with participants in Kilsyth, Ringwood, Knox, Montrose and Glen Waverley. This part-time role is responsible for supporting participants to achieve their personal and NDIS goals, enhance their independence and actively participate in their local community.

What you'll be doing

Work one-on-one with participants to implement individualised support plans that promote community engagement, skill development and independent living

Assist participants with accessing a range of community-based activities, services and programs to foster social inclusion and improve their quality of life

Provide hands-on support with daily living tasks and self-care to enhance participation autonomy and wellbeing

Collaborate with allied health professionals, informal supports, support coordinators and other stakeholders to ensure a holistic, coordinated approach to person centered care

Maintain thorough case notes and documentation to meet regulatory and reporting requirements

Champion a strengths-based, person-centered approach that empowers participants to achieve their goals

What we're looking for

Someone who genuinely would like to help make positive change in a fellow person's life.

Minimum 2 years' experience providing direct support services to people with disabilities

Demonstrated ability to develop positive relationships with participants and implement tasks for their needs

Strong communication and interpersonal skills, with a patient, understanding and considered approach

Proficient in documenting participant progress and maintaining accurate case records

Flexible and adaptable, with a commitment to delivering exceptional, person-centered care

Someone who is wanting regular work with between 1-3 participants on a regular and consistent basis

Current First Aid, current Victorian driver's license, road worthy vehicle, working with children check, NDIS worker screening check, NDIS New worker module certification and willingness to obtain additional training as required (Mandatory)

Qualifications in Aged Care/Disability/Nursing/Community Services and/or Mental Health. Lived experience supporting person or people with disabilities. Knowledge and understanding of the National Disability Insurance Scheme Code of Conduct Rules 2018
