About the Role
The Team Leader oversees the Service Response team receiving day-to-day enquiries and referrals relating to Council home support services delivered under the Commonwealth Home Support Program and Home and Community Care Program for Younger People. The Team Leader will be required to:
• Lead, support and develop the team to build a positive, high-performance culture that delivers optimal client outcomes, excellent customer service and responds flexibly to the changing nature of aged and disability service delivery.
• Lead the team to ensure a consistent, efficient and quality approach to intake, care planning, client monitoring and reviews
• Lead Team to achieve KPI’s and compile and analyse data for outcome measurement and reporting
• Provide support, guidance, coaching and professional development through regular supervision and performance reviews
• Maintain sound knowledge of the needs of clients and implications for the provision of services and supports to diverse communities
Skills & Experience
• Tertiary qualification in human services (or related discipline)
• Experience in leading a team to deliver community-based services to clients
• Excellent communication, interpersonal and problem-solving skills
• Experience with client intake, assessment or care planning
• Demonstrated knowledge of change management principles and use of these to lead through periods of change.
• Knowledge of relevant legislative requirements pertaining to Aged Care and the Aged Care Act
• Proof of Covid vaccinations
