What you'll be doing
Join a dedicated Assertive Community Treatment Team supporting people with severe and complex mental illness through intensive outreach and holistic care planning. Work within a collaborative, recovery-focused multidisciplinary team.
Deliver outreach assessments, treatment and follow-up
Collaborate with clients, carers and community supports
Develop and monitor personalised recovery care plans
Engage in peer review, supervision and ongoing education
Advocate for clients’ rights and support their independence

Essential Requirements:
Qualification: Tertiary qualifications in Nursing
Registration: Full registration with AHPRA
Selection Criteria:
Demonstrated ability to work collaboratively with people with mental illness and families/carers to achieve outcomes that meet consumers’ and families’/carers’ expectations and supports the individual’s desired recovery pathway.
Demonstrated understanding and ability to work within contemporary models of mental health service provision, including the recovery paradigm.
Experience working in an Assertive Community Treatment Team and/or knowledge of the principles that guide service delivery under this model.
Sound working knowledge of the NSW Mental Health Act (2007).
Excellent written, verbal and interpersonal communication skills.
Ability to work within a multi-disciplinary team.
Current NSW driver’s licence and willingness to use for work purposes.
