Role details

The Community Aged Care Assess Team at the Mackay Base Hospital is seeking applications for a Clinical Nurse/Senior Health Practitioner (Physiotherapist, Occupational Therapist or Social Worker) to join their team on a temporary basis.

Your opportunity

As a clinical assessor, you will undertake complex / comprehensive assessments of aged clients in a variety of settings, both community and hospital. Your primary objective will be to record accurate data collected during the complex needs-based assessments of clients using the Integrated Assessment Tool (IAT) within the Aged Care Assessor application and / or the My Aged Care portal. Within a Wellness and Reablement framework, the assessor will develop a client centred Support Plan, detailing client recommendations, including aged care and community pathways to improve service provision and agreed outcomes. The assessor is responsible for providing up to date information on appropriate aged care pathways that the client may be eligible to access. As an assessor you will liaise with clients, carers, health professionals, service providers and other internal and external stakeholders.

About the Team

The Aged Care Assessment Team (ACAT) is transitioning to the Single Aged Care Assessment System. The assessment system is a small, highly skilled multi-disciplinary team providing assessment services to older Queenslanders in the community. The main purpose is to simplify and improve access to aged care services to assist clients to maintain their independence at home, or to enter aged care residential settings. The assessment framework is designed to streamline the assessment process and enhance the overall experience for our older Queenslanders.
