About the Role
The Youth and Family Recovery Worker is a member of the headspace Capalaba team primarily responsible for co-coordinating and delivering recovery orientated care coordination for eligible young people and their families experiencing severe and complex mental health challenges in the enhanced program, headspace Capalaba.
The position will provide centre-based and outreach recovery support and case management working both independently and in partnership with the Senior Mental Health clinician and the clinical team. This role is part of the broader approach to mental health service delivery within a stepped care framework.
The Youth and Family Recovery Worker is required to deliver flexible and responsive non-clinical support, holistic recovery-focused evidence based psychological intervention and supports. The role involves being the key worker and primary point of support for a small case load of families as well as case management services for young people for whom Enhanced clinicians are the primary point of contact. The YFRW will conduct recovery work including assessments, case management, capacity building and holistic support planning and provision of client-centered care. The Youth and Family Recovery Worker is required to attend ongoing and regular case reviews with the Enhanced program as well as the multidisciplinary case review meetings.
Key Role Accountabilities:
Ensure individual and team practice is in accordance with the requirements of relevant legislation including the Human Services Quality Framework and the National Mental Health Standards.
Provide a non-clinical, case management-style support service that uses evidence-based approaches and focuses on identification and intake, needs assessment and support planning, navigation, advocacy and coordination & follow-up.
Work in accordance with psychosocial rehabilitation, social inclusion and recovery-oriented practice frameworks.
Provide a service that complements clinical service provision by supporting engagement with follow-up care and support by working collaboratively with existing services including hospitals and primary care providers
Provide assertive and practical support to access services related to individual needs. Outreach and in home/ community-based support.
Facilitate linkage to ongoing community supports as necessary e.g. primary care providers, community sector agencies (for example, drug and alcohol services, community health, housing/ accommodation services) and to community participation (volunteering), employment or study through local partner connections.
Assist the client to resolve barriers to build strengths and resilience within the community through a recovery-oriented approach equipping them to establish informal support and connections within community groups
Provide support to families and carers, where appropriate, by offering information, encouragement and inclusion in care planning to help strengthen the young person’s natural support networks and promote family inclusive practice.
Provide referrals as appropriate for families/ carers to existing community services that may benefit them including family support services, allied health services and practical support options to provide a holistic lens of support aligned with the collaborate goal planning completed.
About You
Relevant tertiary qualifications and significant experience working with vulnerable families, young people and children.
Demonstrated experience working with “at risk” young people who are experiencing complex needs.
Demonstrated experience using case management principles within strengths based holistic framework.
Demonstrated high level verbal, written communication and interpersonal skills, including case notes, report writing, negotiation, advocacy, and the ability to effectively engage with clients, families and other service providers.
Knowledge of the Community Services sector including a willingness to engage with services to provide collaborative working dynamics and positive outcomes.
Ability to function as a member of a multi-disciplinary team with minimal supervision and capacity to work independently, being accountable for effective delivery of client services.
An understanding of the importance of personal self-care in the challenging work environment demonstrating healthy work boundaries.
Experience working within a recovery orientated service framework for mental health and/or suicide prevention with understanding of the principles of recovery in the mental health setting.
Demonstrated experience working with people with complex support needs across the phases of service delivery including assessment, recovery-oriented practice, community engagement and evaluation of services delivered.
Desirable:
Experience providing care coordination within a youth mental health setting.
