About the Role:
The Homelessness Access Program is a pilot program which aims to enhance primary health care access for individuals experiencing or at risk of homelessness in the Albury and Wodonga areas. By building the capacity of Specialist Homelessness Services (SHS), this program will improve connections between SHS providers and health professionals, ensuring that vulnerable populations receive comprehensive and respectful care.
The Homelessness Health Navigator will be responsible for providing direct support to individuals experiencing homelessness to access primary health services, providing brokerage to get their health needs met, creating resources for staff and clients, and advocating for system improvements.
Selection Criteria:
A relevant tertiary qualification such as Social Work, Nursing, Welfare or Behavioural Science and/or proven relevant experience to this role.
Knowledge of healthcare systems, particularly in addressing the needs of homeless or vulnerable populations.
An ability to adequately assess individual needs, and develop, evaluate, and update care plans in relation to those needs using trauma informed practice.
An understanding of the issues that may lead to a family or individual accessing a health service.
Highly developed communication, networking and interpersonal skills.
