We are currently seeking a passionate and experienced Physiotherapist, who is dedicated to providing therapeutic services to promote our elderly Clients independence and well-being to enable them to remain living in their own homes within Melbourne region, Victoria.

About the Role ...

What you will be doing -

• Assessing our patients/clients with a wide range of acute and chronic disorders and implement effective evidence-based interventions using sound clinical judgement, in the patient's home.
• Work in conjunction with the Case Manager to develop therapeutic services that support healthy ageing and improve our clients' health and well-being.
• Effectively communicate with clients and their families, our staff members and other health professionals, with regards to the provision of physiotherapy services.

About You ...

What you need to succeed -

• Relevant tertiary education in Physiotherapy and current full registration with AHPRA.
• A demonstrated understanding of comprehensive & holistic assessments of consumers to determine an appropriate care plan.
• Ability to plan and carry out physiotherapy services to consumers in need and ensure they are given the appropriate care.
• Ability to continually liaise with families/care givers to ensure ongoing care for the consumer by providing support, counselling and education on consumer management and treatment.
• Capacity to keep detailed treatment records to enable review of clinical activity and ongoing consumer care.
• Ability to develop relationships with individuals from diverse cultural backgrounds
• Excellent customer service skills with proven, effective communication/problem solving skills
• Ability to work autonomously in a community setting
• Demonstrated ability to prioritise workload and meet deadlines
• An understanding and willingness to work with billable targets.
• Commitment to ongoing professional development in the area of aged care physiotherapy
• Current Full First Aid certificate
