The Butler Canberra is seeking a dedicated and compassionate Clinical Partner to join our team. This is an exciting opportunity to make a meaningful impact by supporting older Australians with complex care needs in a values-driven, person-centred environment.
About the role
As a Clinical Partner, you will be a qualified Registered Nurse who delivers and oversees clinical care within your scope of practice. You will work collaboratively with non-clinical care partners, providing complementary clinical support and taking the lead on specialised or complex care needs.
Duties and responsibilities include:
- Complete holistic clinical assessments and develop individualised care plans aligned with each participant’s goals and needs.
- Provide oversight for participants with complex care needs or at key clinical decision points.
- Monitor participant health and wellbeing, identifying and responding to changes in care needs or risks.
- Deliver or coordinate a range of community nursing tasks including:
o Wound care.
o Medication management.
o Continence and falls risk assessments.
o Post-discharge nursing support.
- Engage participants in care planning, health education, advocacy, and informed decision-making.
- Coordinate care and services in collaboration with families, carers, allied health professionals, and community networks.
- Provide guidance and support to non-clinical care staff, including training in participant-specific care requirements.
- Support and strengthen clinical governance across The Butler workforce.
- Maintain effective communication with participants, families, and the broader healthcare team, including GPs.
- Ensure accurate documentation and compliance with organisational and program requirements, including Aged Care Quality Standards.
- Support the team by participating in the after-hours on-call roster on rotation.
About you:
You are a Registered Nurse with a passion for aged care and a commitment to delivering safe, compassionate, and person-centred clinical support. You are confident working both autonomously and as part of a collaborative care team.
Role requirements:
Current Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards.
Experience in care planning, case management or service coordination.
Demonstrated experience in aged care, community care, or primary health.
Ability to communicate effectively with participants, families, and interdisciplinary teams.
Understanding of and commitment to clinical governance and quality improvement.
Excellent communication, organisational and problem-solving skills.
Confidence in managing your time effectively, including an ability to work autonomously.
Police Check, Working with Vulnerable People Card, valid driver’s licence and access to a reliable vehicle.
