Join our team as a Care Partner and play a key role in coordinating services that truly reflect each participant’s needs and choices.

You’ll bring your understanding of participant rights, budget management, and collaborative goal-setting to ensure care is not only compliant but truly personalised.

Working alongside a supportive team, you’ll help deliver outcomes that are meaningful, respectful, and centered on the individual.

What You’ll Do

Understand participant rights and obligations, and establish or review service agreements

Lead person-centred care planning sessions to identify goals, strengths and risks

Design, review and evaluate tailored care plans and quarterly budgets

Coordinate day-to-day service delivery alongside multidisciplinary teams

Monitor quality, timeliness and consistency of all supports provided

Equip participants and families with clear information and self-management strategies

Facilitate timely connections to health, social and community services

Support participants through the process of requesting a Support Plan Review whenever needed

Qualifications & Experience

Essential Criteria

Relevant tertiary qualifications in the Human or Behavioral Sciences, Allied Health, Aged Care or Community Services Fields

Experience in aged care client case management

Knowledge of new aged care regulatory and legislative frameworks and requirements

Proven ability to connect with others through clear communication, empathy, and a commitment to outstanding service.

National Police clearance

Driver’s licence
