About the role
The Day Program Facilitator is responsible for planning, delivering, and evaluating a range of centre-based and community-based activities to promote social engagement, independence, and wellbeing of older people, including those with dementia or mobility challenges. The role requires a client-centred, compassionate, and proactive approach, ensuring all activities are inclusive, engaging, and aligned with the individual needs and care plans of clients.
This is a remotely based role, and you will be required to travel within the NSW.
Key Responsibilities
Activity Facilitation
Lead and facilitate diverse group and individual activities tailored to clients’ interests, abilities, and cultures, promoting social engagement and independence in a welcoming environment.
Program Planning and Development
Assist in designing and scheduling monthly activity calendars that reflect client feedback and wellness goals, incorporating person-centred care and therapeutic approaches.
Client Care and Engagement
Support clients’ physical and emotional wellbeing, including assistance with mobility, personal care, meals, and medication prompts, while building positive relationships and monitoring client conditions.
Compliance, Safety, and Documentation
Ensure program delivery aligns with WHS, infection control, and aged care standards, while maintaining accurate records, reporting incidents, and upholding confidentiality.
Transport and Excursion Support
Coordinate and support safe client transport and outings, managing logistics, accessibility, and risk to ensure enjoyable and inclusive external activities.
Qualifications & Experience
Essential:
Certificate III or IV in Individual Support, Aged Care, Community Services, Leisure and Health or equivalent.
Demonstrated experience in aged care, disability support, community recreation or group facilitation.
Understanding of the Aged Care Quality Standards and person-centred care.
Current First Aid and CPR certificate.
NSW Driver’s Licence (C Class) and willingness to use own or fleet vehicle as required.
Current National Police Check and NDIS Worker Screening Check (or willing to obtain).
Desirable:
Experience supporting people with dementia or cognitive impairment.
Experience working with diverse cultural groups or delivering culturally appropriate activities.
Knowledge of wellness, reablement, or restorative care models.
