About the Role
We’re looking for an experienced Care Coordinator to provide person-centred virtual care to our members navigating complex healthcare journeys. You’ll be the trusted point of contact, helping members access the right services, manage their care needs, improve their self-management and overall wellbeing.
Key Responsibilities
- Provide one-on-one telephonic care coordination for members with chronic or complex medical conditions
- Conduct initial assessments and develop tailored care plans
- Collaborate with healthcare providers, hospitals, and allied health professionals
- Monitor progress, offer guidance, and adapt care plans as needed
- Educate and empower members to make informed health decisions
- Ensure smooth transitions between different levels of care (e.g. hospital to home)
What You Bring
- Clinical background (e.g., Registered Nurse, Allied Health Professional, or equivalent)
- Experience in care coordination, case management, or chronic disease management
- Strong understanding of the Australian healthcare system and referral pathways
- Excellent communication, empathy, and organisational skills
- Ability to work independently and collaboratively within a multidisciplinary team
