About the Role
You'll be working closely with the Home Care Manager and a multidisciplinary team to deliver high-quality physiotherapy services to older adults funded through Home Care Packages (HCPs). Your responsibilities will include:

• Conducting person-centered physiotherapy assessments
• Developing and reviewing tailored treatment plans
• Recommending assistive technology and home modifications
• Collaborating with families, caregivers, and other providers
• Supporting education and training for care staff

What You'll Need

• Bachelor of Physiotherapy and current AHPRA registration
• Minimum 2 years of experience in community or aged care settings
• Strong clinical reasoning and communication skills
• Knowledge of wellness and reablement models
• Confidence prescribing equipment and mobility aids
• Current Driver's Licence and reliable vehicle
• NDIS Worker Screening Check and WWCC (or willingness to obtain)
