At Enabling Pathways, we believe in empowering lives and enabling futures. We're on the lookout for a passionate, skilled Outreach Case Manager to join our Townsville-based Child Safety team.
You'll deliver hands-on, trauma-informed support to young people under Child Safety Orders—including outreach to homes, services, and community spaces. You’ll provide consistent, youth-centered case management, collaborate with key stakeholders, and walk alongside young people as they navigate permanency, stability, or transition to independent adulthood.
What You’ll Be Doing:
As a Case Manager, you will support young people to meet their immediate needs and help build a sense of stability and connection. Your role includes:
- Providing emergency outreach support to young people in the community
- Assisting young people to access essential resources and build safe, caring relationships
- Supporting engagement with education, employment, or training opportunities
- Helping to maintain and strengthen positive peer, family, and cultural relationships
- Supporting young people in their journey toward adulthood and independence
- Developing and monitoring case plans in partnership with Child Safety, families, and the young person.
- Providing wraparound support for wellbeing, safety, cultural connection, housing, education, legal, and health needs.
- Embedding harm minimisation principles and life skills into everyday practice.
- Maintaining high-quality documentation, Safety planning, progress notes, assessments, and compliance reporting.
- As an integral part of the care team you will be required to attend team meetings, external stakeholder , and multi-agency planning sessions.
Skills & Experience:
- A qualification in Social Work, Psychology, Youth Work, or a related field.
- Minimum 2 years’ experience in Child Protection or complex-needs youth case management.
- Ability to communicate effectively
- Excellent communication (written and verbal) and interpersonal skills, including working and communicating effectively with clients, their families, community agencies and other professionals.
- Ability to work in a busy environment, both independently and as a part of a team.
- Comply with & understand all privacy and confidentiality policies.
- Cultural awareness of all backgrounds.
- Solid knowledge of Child Protection Orders, case planning processes, and relevant QLD legislation.
- Confidence in crisis response, risk assessment, and culturally responsive practice (particularly with Aboriginal and Torres Strait Islander young people)
- Ability to work independently and flexibly in community-based outreach settings.
- Ability to travel outside of Townsville Region is a requirement
Mandatory Requirements:
- Proof of citizenship (passport, birth certificate or citizenship certificate) or VISA if applicable.
- Current QLD Open Driver's License.
- Must hold a QLD Working with Children Blue Card.
- Hold a current First Aid (HLTAID011) and CPR (HLTAID009) Certificate.
- A clean National Police check less than 6 months old.
- PeakCare Training Certificates: Hope and Healing for Residential Care, Positive Behaviour Support & Managing High Risk Behaviours, Water Safety Awareness
- Infection Prevention and Control (PPE) Training
- Minimum Certificate IV in Child, Youth and Family Intervention or equivalent
- Willing to undergo training in specific courses to support our client’s needs
