About the role
Permanent part time opportunity to deliver high level Social Work clinical services, including proactive consultation, management, and oversight of existing and potential consumers whose complex psychosocial and disability care needs may delay or impede timely discharge.
Provide advice and direction to COH clinical teams where reasonable adjustments may be required to meet the individual needs of consumers with disability, in alignment with Queensland Health's Reasonable adjustments for patients with disability guideline and Metro North Health's Disability Services Action plan 2024-2029.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values::
- Provide high level Social Work consultation to Social Workers, a range of health professionals, Supervisors, relevant Service Managers and other stakeholders on the management of care for consumers with disability.
- Utilise high level communication and advocacy, negotiation and conflict management skills to engage with consumers and their families/carers, other health professionals and stakeholders to secure access to resources required for optimal consumer outcomes.
- Provide professional leadership in developing and implementing evidence-based processes and interventions to support and optimise access to services and timely discharge outcomes for consumers, exploring alternative courses of action to modify or change clinical practices, in collaboration with the COH Social Work service, other health professionals, COH Service lines, and key stakeholders across MNH.
- Apply high level clinical knowledge of disability pathways, where principles and procedures may need constant adaptation or modification to meet individual needs, clinical requirements and legal standards.
