Mark & Sylvie’s Home Care is committed to supporting individuals to live with dignity, independence, and purpose in their own homes. We are currently seeking a qualified and compassionate Social Worker to support our clients across Hastings and Bangholme.
This is a dynamic role providing both immediate support for acute situations and ongoing assistance for clients with complex needs. Support may be delivered through home visits, telehealth, and meaningful contributions to clinical meetings and multidisciplinary hubs.

About You:
Diploma or Degree in Social Work, Community Services, or Mental Health (or equivalent)
Experience supporting clients with complex needs in aged care, home care, or community health settings
Strong communication and relationship-building skills
Comfortable providing support via both in-person visits and telehealth
Ability to work autonomously and contribute within a team
Current driver’s licence and reliable transport
