Founded in 2010 through strong community collaboration, MKAC operates as a culturally safe and holistic ‘one-stop shop’ for wellness support, community programs, and advocacy.
About the Role
We are seeking a compassionate, proactive and community-minded individual to join our Elder Care Support Program as an Aged Care Connector (ACC).
In this vital role, you will support Aboriginal and Torres Strait Islander Elders and their families to navigate aged care pathways, including My Aged Care, with cultural safety and respect. You will help identify service needs, connect clients to local aged care providers, and advocate for appropriate and sustainable care.
Key Responsibilities
Build trusted relationships with Elders, families, and local Aboriginal communities.
Raise awareness and provide information on My Aged Care processes.
Support clients with pre-assessment and aged care registration, including assisting with paperwork.
Attend assessments, contribute to care plans, and help review services.
Conduct regular home visits and community outreach to identify potential clients.
Support ongoing advocacy and link clients to additional services as needed.
Maintain accurate client data and contribute to program development and reporting.
Participate in team meetings, stakeholder discussions, and quality improvement initiatives.
What We’re Looking For
Essential:
Strong communication and interpersonal skills, especially with Aboriginal communities.
High reliability, empathy, and a positive, client-first mindset.
Ability to work independently and collaboratively in a team.
Willingness to learn aged care systems, pathways, and service planning.
Understanding of Aboriginal culture, history, and trauma-informed care.
Valid WA Driver’s Licence, Police Clearance (within 3 months), Working with Children Check (or ability to obtain), and COVID-19 vaccination.
Desirable:
Existing relationships with local Aboriginal and Torres Strait Islander communities.
Experience in aged care, community services, or health roles.
Knowledge of My Aged Care operational processes.
Qualifications in Community Services or a related field.
First Aid and/or Aboriginal Mental Health First Aid certification.
