We are seeking an experienced and compassionate Care Partner to join our team on a part-time basis (flexible hours for the right candidate). We understand the importance of work-life balance and this part time role offers flexible hours that can work around school drop-offs, study, or other life commitments. This hybrid role offers the best of both worlds: working from home and providing face-to-face care partnership across the Geelong region.

About the Role

As a Care Partner, you’ll provide tailored support to older Australians who want to stay living independently in their own homes, supported by services that reflect their goals and choices. You’ll oversee the delivery of Home Care Packages (HCPs), advocate for clients' needs and ensure services are coordinated efficiently, compassionately and in line with evolving aged care reforms, including the upcoming Support at Home program.

Your Day Might Include:

Getting to know participants and understanding what matters most to them

Conducting assessments and creating individualised care plans

Coordinating services and maintaining quality care delivery

Visiting participants in their homes across Geelong

Advocating for clients and navigating the aged care system with them

Monitoring wellbeing and updating care plans

Ensuring compliance with aged care standards and Real Caring policies

What You’ll Bring:

Experience with Home Care Packages (essential)

Understanding or interest in the Support at Home program (desirable)

Relevant qualifications in Community Services, Aged Care, or Allied Health

A person-centred mindset with empathy, integrity, and adaptability

Strong coordination and communication skills

A current driver’s licence and access to a vehicle
