About the role
As Transform NDIS Support continue to grow, we are looking for a passionate and experienced Disability Support Workers to join our team in multiple locations across greater, northern and southern suburbs of Adelaide for a full-time position.
Key Responsibilities
To support our clients by implementing acquired skills to support clients with day-to-day activities.
To assist clients in rebuilding and retraining in skills of daily living, including personal care, domestic assistance, social/community support, transport and completing support/shift notes to compliance standards.
To perform direct care duties for individuals’ clients as specified in the personal care profile.
To carry out designated duties/tasks as per the service duty roster and line managers instruction.
To support clients to establish and maintain relationships and interest through access to community activities, venue and services.
To take case notes of every shift and maintain adequate records on the client’s progress.
To communicate in a timely manner any incidents during the shifts.
About You
You must have demonstrated understanding of a person-centred approach, good verbal and written communication skills, the ability to connect with others and ability to follow individual support care plans.
Skills, Qualification and Experience
Have at least 1 year experience and Certificate III/ Certificate IV/diploma in Disability and/or Mental Health and/or 2 years relevant work experience to substitute the qualification.
Current First Aid & CPR (or willingness to obtain).
Current and valid Driver’s License & Registered Vehicle.
Current Working with Children Check.
Current NDIS Worker Screening Check.
MAPA training not mandatory but desirable.
Demonstrated understanding of a person-centred approach.
Good verbal and written communication skills and the ability to connect with others.
Ability to follow individual support care plans.
Fluency in English highly desirable.
