We began 24 years ago with a knock-out exhibition at the Art Gallery of NSW which raised over a million dollars. People from the remotest, most beautiful part of the desert, then used this money to develop their own organisation, with a culture of caring, compassion and determination.

At the time, the focus was to get family members home to Kintore on dialysis, for people to be looked after on country and to maintain the traditions of looking after country and passing on cultural heritage to future generations.

It took a little time…and a lot of hard work…but in 2004, Purple House started dialysis in Kintore, the first of its kind, and set a precedence for many others to follow.

Today we are proud to say we have helped many communities to get their people home and now operate Dialysis in 20 (and growing) remote communities across NT, SA and WA. We also run two mobile dialysis trucks, provide a range of support services including Aged Care, NDIS, Primary Health and Social Support, run a successful bush medicine social enterprise and always working on new innovations.

We have come a long way in two decades and are excited about the next chapter!

About the Role

As the Last Look Project Coordinator, you will be responsible for coordinating and refining the Last Look program, which supports safe and culturally appropriate transport for people living with chronic kidney disease who are returning to their remote communities for a final visit or for end-of-life care.

This hands-on role involves program development, stakeholder engagement, staff training, logistical planning, and ensuring the physical and emotional wellbeing of patients and staff. You’ll also help translate standards into best practice while identifying opportunities for continuous improvement.

Key Responsibilities

Program Development & Coordination

Develop formal policies, procedures, and risk management strategies for the Last Look program to ensure the safety of both patients and staff.

Establish clear referral pathways and criteria for Last Look trips, collaborating with both internal and external stakeholders.

Work closely with NT Government (NTG) palliative care teams, remote Primary Health Care clinics, NTG/Purple House renal services, nephrologists, and GPs to coordinate patient-centred Last Look trips.

Develop culturally appropriate processes for Advance Care Planning (ACP), beginning these discussions early in the patient’s dialysis journey.

Liaise with families and support networks to explore end-of-life care options that align with the patient’s wishes.

Staff Recruitment, Training & Support

Recruit and support both clinical and non-clinical staff interested in participating in Last Look trips.

Develop and deliver training programs in collaboration with Occupational Therapists and other stakeholders, ensuring minimum training standards are met.

Provide staff with ongoing support, including debriefing sessions and mental health referrals as needed.

Mentor support workers to help them confidently lead ACP discussions and family meetings.

Stakeholder Engagement & Networking

Build strong working relationships with GPs, palliative care teams, renal services, remote PHC staff, aged care and NDIS teams, and remote dialysis nurses.

Identify and apply for funding opportunities to sustain and grow the Last Look program through grants and partnerships.

Collaborate with organisations such as PEPA, Menzies, ARIIA, and EASA to enhance training and contribute to relevant workshops and projects.

Investigate research collaborations with universities and research institutions to build the evidence base for the Last Look program and improve practice.

Operational & Equipment Management

Oversee the maintenance and servicing of the Last Look vehicle and related equipment with the support of the operations team.

Create and implement processes for trip approvals, contingency planning, risk assessments, and safety protocols.

About You – Essential Criteria

To be successful in this role, you will have:

A minimum Certificate III in Aged Care or a similar qualification (e.g. Community Services, Disability Work)

Experience in mentoring and training staff

A general understanding of community aged care services and key standards in service delivery and case management

Cultural sensitivity and the ability to empathise with clients, advocating for their needs

Strong administrative and organisational skills, including computer literacy and report writing

Excellent communication, conflict resolution, negotiation, and problem-solving skills

The ability to manage multiple priorities and adapt quickly when challenges arise

A willingness to work in a cross-cultural environment, under Indigenous leadership

A commitment to quality improvement and knowledge of current Work Health and Safety legislation

A current Australian driver’s licence

Preferred Qualifications & Experience (or willingness to complete training)

Clinical experience working with dialysis patients and/or palliative care

Mental Health First Aid certification

Ambulance stretcher training

Completion of PEPA (Program of Experience in the Palliative Approach) placement or training
