About the Role
Reporting to the CEO and Executive Leadership Team, the Head of Community Care Services will lead the strategic, operational, and clinical delivery of high-quality, consumer-directed community aged care services. You will drive digital transformation, workforce capability, service growth, and compliance with the new Aged Care Act and Strengthened Quality Standards.
This pivotal role will shape Pennwood’s future service model in line with the Support at Home reforms and the broader transformation of community-based aged care.
Key Responsibilities
• Provide visible leadership across home care, CHSP, transitional, and restorative services, ensuring continuity, integration, and sustainability
• Lead a multi-disciplinary team across clinical, care coordination, administration, and scheduling functions
• Champion workforce capability, succession planning, and a performance-driven culture of service excellence
• Ensure clinical governance, quality assurance, and operational compliance with all legislative and regulatory obligations
• Translate strategic direction into measurable service outcomes and financial performance
• Actively lead digital maturity, innovation pilots, and data-driven service improvement
• Collaborate with internal leads including Corporate Head of Quality and Innovation and People and Culture Manager to ensure aligned strategic outcomes
• Oversee incident management, audits, consumer feedback, and continuous improvement initiatives
• Represent Pennwood in regulatory, sector, and community forums
What We’re Looking For
Essential:
· Proven Leadership of Multi-Disciplinary Teams
Demonstrated ability to lead, mentor, and develop complex clinical and non-clinical teams in aged care, health, or community service environments.
· Deep Knowledge of Aged Care Legislation and Reform
Comprehensive understanding of the Aged Care Act, Support at Home reforms, Strengthened Aged Care Quality Standards, Serious Incident Response Scheme (SIRS), and Open Disclosure obligations.
· Risk Identification and Operational Governance
Demonstrated capability to identify and respond to clinical, operational, and systemic risks through robust governance frameworks and ethical decision-making.
· Strategic Execution and Service Optimisation
Proven track record in translating organisational strategy into service delivery outcomes, embedding reform, and driving continuous improvement across community-based aged care.
· System and Process Development
Experience designing and embedding scalable, compliant systems and processes to support rostering, case management, and care coordination.
· Financial Acumen and Performance Management
Ability to manage divisional budgets, interpret performance data, meet KPIs, and contribute to grant applications and sustainable service expansion.
· Client and Stakeholder Engagement
Strong interpersonal skills with the ability to manage complex client matters, build sector partnerships, and represent the organisation credibly in forums and negotiations.
· Digital and Data Competency
High proficiency in Microsoft 365 and digital care platforms (e.g., Leecare, e-Tools), with a demonstrated ability to use data to inform quality, planning, and optimisation.
· Communication and Interpersonal Skills
Exceptional verbal and written communication skills, with the ability to influence stakeholders, lead change, and resolve sensitive issues with professionalism and empathy.
Desirable:
· Registered Nurse (Division 1 – AHPRA)
· Tertiary or postgraduate qualifications in aged care, health administration, leadership, or a related field
· Experience in transitional care, restorative care, or Support at Home service models
