This full-time opportunity is for a Team Leader, Lifestyle & Wellbeing to join our Toowoomba Home and Community Service.

About the role:
The Team Leader will support the delivery of community services with a particular focus on lifestyle support programs utilising the principles of social inclusion, choice and control, enablement, empowerment and independence.

The Team Leader will supervise a team of Lifestyle Assistants on a day-to-day basis, assist with orientation of new staff and provide guidance and support including assessments, client related documentation and other related activities as delegated.

Other responsibilities include but are not limited to:

• Planning, co-ordinating and evaluating non-clinical community based services
• Facilitating the support needs of clients through implementation of individualised care plans
• Providing operational support and guidance to the team and volunteers to support service delivery

Minimum qualifications:

• Current National Police Check
• Current Drivers Licence and own reliable vehicle
• Degree or Diploma in Diversional Therapy or Certificate IV in Leisure and Health, or equivalent
• Current First Aid and CPR Certification
