About the Role
We are seeking a House Leader (Registered Nurse) to lead the day-to-day operations of a Supported Independent Living (SIL) home, supporting clients with chronic and complex disability and health needs. This full-time, hands-on role blends clinical nursing responsibilities with house coordination and leadership, ensuring consistent delivery of high-quality, person-centred care.
You will coordinate and guide a dedicated team of support workers, oversee daily routines, and work closely with Client Liaison Officers to manage appointments, community participation, and client support. With strong attention to detail, you’ll maintain accurate records, ensure compliance with documentation standards, and support incident reporting processes.
Your clinical expertise will be central to delivering excellent care, while your leadership will foster a collaborative and supportive team culture. You will work closely with the Clinical Lead and engage with other Breakaway service streams to ensure client goals are met, documentation is maintained, budgets are monitored, and quality outcomes are achieved.
Your Key Responsibilities:
- Lead with Purpose: Oversee the day-to-day coordination of a Supported Independent Living (SIL) home, providing guidance and support to your team.
- Deliver Clinical Excellence: Provide direct nursing care including medication administration, health monitoring, and responding to complex clinical needs.
- Support Client Wellbeing: Work closely with the Clinical Lead and Client Liaison Officers to coordinate appointments, monitor goal achievement, and ensure person-centred care.
- Maintain Accurate Records: Ensure all documentation—including progress notes, incident reports, and care records—is completed to a high standard and kept up to date.
- Promote Positive Behaviour Support: Implement and oversee Positive Behaviour Support Plans and manage risks in line with NDIS and organisational guidelines.
- Build Trusted Relationships: Serve as the key contact for clients and families, creating a supportive environment built on trust, transparency, and respect.

- Registered Nursing Expertise: AHPRA-registered nurse with proven experience in disability, community care, or complex health support.
- Commitment to Best Practice: Strong knowledge of NDIS standards, risk management, and clinical compliance requirements.
- Communication & Collaboration: Excellent interpersonal skills with the ability to work effectively with clients, families, teams, and external stakeholders.
- Organisation & Flexibility: Able to juggle multiple priorities while staying calm, adaptive, and solution-focused in a dynamic environment.
What We’re Looking For
- AHPRA Registered Nurse – current and in good standing
- Proven experience in disability support, aged care, or chronic condition management
- Experience with Enteral Feeding desirable
- Strong interpersonal and communication skills
- High standard of professionalism, integrity, and person-centred care
- Ability to assess, respond, and make sound clinical decisions
- Flexible, team-oriented, and values-driven
- Current CPR Certificate
- Current Blue Card and NDIS Worker Screening (or willingness to obtain)
- Valid Driver’s Licence (minimum Provisional)
