Mental Health Clinician - Case Management (Key Clinician)
Location: Melton - Outer Community Team, Western Health Mental Health & Wellbeing Service
Classification: RPN3 / OT2 / SW2
Employment Type: Full Time
Work Schedule: Monday to Friday, 8:30 AM to 5:00 PM (40 hours per week) with an Accrued Day Off (ADO)
We are seeking a dedicated Mental Health Clinician to join our Outer Community Team at Western Health. This is a key clinician role, responsible for the coordination and delivery of high-quality, recovery-oriented mental health care to a group of allocated consumers living in the community.
You'll be part of a supportive, multidisciplinary team committed to providing evidence-based, person-centred care to consumers with complex mental health needs. Our service operates across the growing communities of Melton and parts of Brimbank, offering both challenge and opportunity.
What You'll Be Doing
- Act as the key clinician for a caseload of consumers with serious mental illness
- Provide holistic, recovery-focused assessment, planning, intervention, and review
- Engage with families, carers, and community supports to enhance continuity of care
- Support consumers through periods of crisis, transition, and stabilisation
- Collaborate with internal teams and external agencies to advocate for consumer needs
- Maintain high-quality clinical documentation using electronic systems
- Contribute to team portfolios, service development, and quality improvement
What You'll Bring
- Minimum 2 years' experience in a mental health clinical setting, ideally within a community-based service
- Qualifications and current registration as one of the following:
- Registered Psychiatric Nurse (Division 1)
- Occupational Therapist
- Social Worker - Knowledge of the Mental Health and Wellbeing Act (2022) and recovery-oriented practice
- Excellent interpersonal, clinical, and documentation skills
- Victorian Driver's Licence

Position Description
About Western Health
Western Health manages three acute public hospitals: Footscray Hospital, Sunshine Hospital - including the Joan Kirner Women’s and Children’s - and the Williamstown Hospital. It also operates the Sunbury Day Hospital, and a Transition Care Program at Hazeldean in Williamstown. A wide range of community based services are also managed by Western Health, along with a large Drug and Alcohol Service.
