Management and Accountability

Direct accountability is to the Manager of Home Care and Support Services

Key Responsibilities

Clinical Assessment and Intervention

Conduct thorough physiotherapy assessments for older adults, considering age-related changes, comorbidities, and functional capacity.

Develop and implement individualized, goal-oriented treatment plans that enhance mobility, reduce pain, prevent falls, and support independence in daily living.

Deliver high-quality 1:1 physiotherapy services in clients' homes, or community settings.

Manage musculoskeletal, orthopaedic, and chronic health conditions common in older populations, such as arthritis, osteoporosis, COPD, cardiovascular disease, neurological conditions, and post-surgical rehabilitation.

Rehabilitation and Falls Prevention

Provide tailored exercise programs to improve strength, balance, and functional mobility, both individually and in group settings.

Educate clients, families, and carers on safe mobility practices, manual handling, and strategies for falls prevention.

Prescribe and coordinate the provision of mobility aids, assistive equipment, and home modifications to support safe, independent living.

Empower clients, their representatives and care givers with knowledge about mobility strategies and rehabilitation techniques.

Multidisciplinary Collaboration

Work collaboratively with care staff, nurses, GPs, allied health professionals, and external providers to ensure integrated, holistic care for aged care clients.

Participate in case conferences, care planning meetings, and communication with families and support teams as needed.

Collaborate with AMCS’s leadership team to refine and expand service offerings

Support and Supervision

Provide guidance, task delegation, and supervision to Allied Health Assistants in aged care service delivery.

Contribute to the supervision, training, and development of physiotherapy students and interdisciplinary student placements.

Compliance and Documentation

Maintain accurate, timely, and confidential documentation of assessments, treatment outcomes, and client progress in accordance with organisational, aged care, and privacy standards.

Ensure adherence to the Aged Care Quality Standards, AHPRA requirements, and relevant legislation, policies, and funding guidelines.

Continuous Improvement and Quality Care

Participate in continuous quality improvement initiatives and service evaluations aimed at enhancing physiotherapy and aged care outcomes.

Support accreditation processes and assist in implementing evidence-based practice improvements across services.

Key Selection Criteria

Essential Qualifications & Experience:

Tertiary qualification in Physiotherapy

Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)

Demonstrated experience in community-based or aged care physiotherapy (2 years)

Knowledge and understanding of the needs of seniors, particularly those from CALD backgrounds

Experience with development, implementation and evaluation of groupexercise programs.

Valid driver's licence and access to a reliable vehicle with a minimum of third p[arty care insurance

National Police Check and NDIS Worker Screening (or willingness to obtain)
