The Warracknabeal campus—where this position is based—offers a broad range of acute, urgent care, aged care, and allied health services and is ideally located halfway between Melbourne and Adelaide.

In this rewarding role, you’ll provide support that empowers clients to maintain their independence and remain actively involved in their care. This community-based role involves travelling between customer's homes and delivering personalised, day-to-day support aligned with individual care plans.

About You:

Certificate III in Aged Care, Home & Community Care, Disability, or equivalent

Ability to understand and follow individual care plans

Strong communication skills with clients, families, and caregivers

Relevant experience in home or community care settings

Current First Aid Certificate

Current Victorian Driver’s Licence

Evidence of current flu vaccination

Valid Working with Children Check or willingness to obtain

Current Police Check

NDIS Worker Screening Check or willingness to obtain
