👉🏼 The need-to-know…

Rise is seeking the cream of the crop (aka the very best!) Support Workers for our North Metro community clients. We have available shifts on weekdays with shift times 8am to 1pm, additionally availability is desirable. Community travel to multiple locations per day to client homes will be required in and around Wanneroo, Karrinyup and Balcatta. Travel time & kilometres between clients will be reimbursed.

Day to day, you will follow client support plans, record/report changes, contribute to programme meetings, and assist clients with activities of their daily living whilst achieving their personalised goals. Responsibilities may also include domestic cleaning duties.

👉🏼 The need-to-have…

Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following:

Certificate III in individual support (Aged) or equivalent with demonstrated experience post-studies

First aid certificate (HLTAID009 and HLTAID010) or willingness to obtain

Comfortability with providing personal care and essential services

Driving licence with use of a reliable vehicle for work purposes

Availability on weekdays (minimum two days) for shifts 8am to 1pm

✅ Formalities (The serious stuff!)

Appointments to this position will be subject to a national police clearance and pre-employment medical assessment. Costs for new checks will be covered and/or reimbursed by Rise.

A cover letter is recommended within your application. Within your letter, please address the following:

Outline your Aged Care Community experience including medication support and providing personal care.

One example of how you have previously supported a client with Dementia.
