Position Summary: As a Community Care Worker, you will play a vital role in delivering personalized care and support to individuals in their own homes or community settings. You will work closely with clients to help them maintain their independence and improve their quality of life through a range of services tailored to their specific needs.

Key Responsibilities:

Provide personal care and support to clients, including assistance with daily living activities such as bathing, dressing, and meal preparation.

Support clients with medication management, ensuring that they follow their prescribed treatment plans.

Assist with mobility and transfers and provide companionship to enhance clients' emotional well-being.

Develop and maintain strong, trusting relationships with clients and their families.

Collaborate with healthcare professionals and other service providers to ensure coordinated care.

Document and report changes in clients' conditions and any concerns to the appropriate parties.

Adhere to all safety, hygiene, and privacy protocols.

Qualifications:

Certificate III Individual Support, Aged Care, Disability or equivalent e.g 2nd year Bachelor or Nursing

Experience working in aged care, disability or community nursing is desired

Current First Aid and CPR certificates

National Police Clearance

The right to work in Australia

Previous experience in a caregiving or healthcare role is advantageous.

Strong interpersonal and communication skills.

Compassionate, patient, and respectful approach to care.

Ability to work independently and as part of a team.

Reliable transportation, Comprehensive car insurance and valid driver’s license.
