The Quality and Innovation team provides clinical oversight and support to IUIH member services and is responsible for the service development and delivery of urban Aboriginal and Torres Strait Islander health and family wellbeing care. This diverse team includes IUIH Data and Research, Logistics, Oral Health, Pharmacy, Policy, Social Health, Primary Health Care Services and Regional Clinical Support.
About the role?
The Primary Health Care Home Visiting Nurse is responsible for delivering a culturally appropriate nursing service for mob, our clients and families at home across the lifespan, in South East Queensland. This position is a key part of the Primary Health Care Service and works in collaboration with a multidisciplinary team to support clients in various sectors, including, but not limited to Mob Link, Palliative Care, and Aged Care. The role involves providing nursing support in aged care, episodic and end-of-life care, and in training and information delivery in alignment with the IUIH Model of Care.
Day to day duties will include:
- Provide day to day health service activities within the context of Comprehensive Primary Health Care in a professional, confidential and culturally safe manner.
- Perform service delivery within scope of practice and within own personal experience and ability, observing all standard clinical practice policies and procedures.
- Provide routine screening and early detection of health conditions through initiation and performance of all aspects of comprehensive health assessments, other than areas identified as requiring the GP for completion.
- Managing a rolling case load of clients requiring a diverse spectrum of health conditions including, but not limited to palliative care and aged care
- Triage and prioritise referrals, coordinate home visiting nursing care
- Work with clients primary health care team to ensure wrap around care is provided in line with the IUIH ways.
- Enter any recalls and reminders issuing from the consultation into the electronic health record system.
- Ensure information is recorded accurately and in a timely manner in the electronic health record system, and that records are maintained in accordance with AGPAL standards.
- This role involves travelling to any IUIH member services across SEQ
About You?
- Current AHPRA Registration (Registered Nurse)
- Minimum of 5 years experience in a similar role
- Current C Class Drivers Licence (QLD)
- Current CPR (willing to obtain)
- Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander communities, their leaders and health professionals, while respecting traditional culture, values and ways of doing business.
- An ability to communication and empathise with Aboriginal and Torres Strait Islander people
- Ability to work as part of a multi-disciplinary team and understand the cycle of care patient system
