About the Role
AHP-1 Join the Central Adelaide Local Health Network (CALHN) Mental Health Clinical Program, where you will be part of a multidisciplinary team delivering recovery-focused clinical services across inpatient, sub-acute, community, and rehabilitation settings. As a Mental Health Clinician – Social Worker, you will play a crucial role in improving clinical outcomes for consumers of all ages, from youth to older persons. You will be responsible for care coordination and the delivery of a broad range of clinical activities, including acute mental health and psychosocial assessments, care planning, and clinical reviews, all in line with CALHN’s Model of Care and Service Plan.
Working collaboratively with consumers, families, and carers, you will provide specialist psychosocial and family assessments, crisis interventions, rehabilitation services, and assertive clinical support to promote wellbeing and stabilise mental health. You will also build strong partnerships with a range of stakeholders to ensure coordinated, high-quality care. AHP-2 As a valued member of the integrated multi-disciplinary team within the community mental health service, the Social Worker plays a crucial role in delivering high-quality, recovery-focused care. You will be responsible for care coordination, care planning, clinical reviews, psychosocial and mental health assessments, all delivered in line with Clinical Business Rules. Working collaboratively with consumers and their families or carers, you will focus on supporting mental health stabilisation, enhancing wellbeing, and building consumers’ capacity for self-management, enabling them to engage in meaningful community activities and improve their overall functioning.
In this dynamic role, you will provide specialist assessments, therapeutic interventions, crisis management, advocacy, and family/carer support, while maintaining strong connections with key stakeholders, including primary care providers, other mental health teams, and government and non-government organisations. You will also have the opportunity to contribute to service improvements, evaluate clinical services, and engage in professional development and research activities.
