We’re expanding our team and are looking for Enrolled Nurses and Student Registered Nurses (RNs) to join our Aged Care Assessment team. This role offers the opportunity to use your clinical skills in an office-based, Monday-to-Friday environment with no shiftwork, no weekends, and hybrid options available after probation. You’ll focus on aged care assessment and planning.

What you’ll do.. Working in a high tech, telehealth environment
• Conduct personalised assessments for new and existing Home Care Package clients
• Translate needs into clear, client-focused care plans using enablement frameworks
• Recommend services and supports tailored to individual lifestyles and goals
• Document and update assessments using digital care systems (we’ll train you)
• Work collaboratively with your team, clients and families

• Experience in aged care, disability, or health services is valued but not required
