About us:
Sub-Acute Services provides support to the Rehabilitation and Sub-Acute Geriatric Evaluation Units at Rockhampton Hospital. These are specialised areas of care committed to restoring and maintaining the optimum level of function of the individual and significant others, enabling them to achieve their lifestyle goals.
About the role:
Utilise high level nursing competency to coordinate patient flow through the Rehabilitation, Sub-Acute Geriatric Evaluation Unit (SAGE) and Huxham Units and improve the client journey generally throughout the Central Queensland (CQ) Health.
This role will provide comprehensive skills of coordination and the delivery of client-centred care. It will minimise readmission and will provide clinical skills and intervention within Sub-Acute Ambulatory and Outreach models which may include outpatient clinics to ensure appropriate transition for ongoing client management.
About you:
- Minimum three years' experience in general nursing across acute and community domains is highly desirabl
- Current registration as a Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current practising
- While not mandatory, a relevant post graduate qualification, or working towards, in Rehabilitation or Geriatric Evaluation Nursing would be highly desiarable
What we offer:
Total monthly value of up to $5,712 p.m. is comprised of:
- An hourly rate between $54.44 - $58.30 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
