About us:
Transition Care Program provides short-term care that seeks to optimise functioning and independence of older people after a hospital stay. TCP is goal-orientated, time-limited and therapy focused. It provides older people with packages of services that include low intensity therapy, such as physiotherapy and occupational therapy as well as social work, nursing support or personal care. It seeks to enable older people to return home after a hospital stay rather than enter residential care prematurely.
About the role:
- Case management of clients for service provision while they are on the community rehabilitation program.
- Provide Clinical Rehabilitation Therapy to clients who are on the community rehabilitation program.
- Provide specific Physiotherapist / Occupational Therapy activities to the wider Multidisciplinary Team (MDT) Clinicians clients.
- Provide Clinical Supervision, support, and delegate client therapy programs to the Allied Health Assistant in the team.
- Undertake quality activities within the team.
About you:
Appointment to this position requires proof of qualification as a registered Physiotherapist / Occupational Therapist and maintain the registration requirements with the Australian Health Practitioner Regulation Agency (AHPRA), and the Physiotherapist / Occupational Therapy Registration Board of Queensland including any necessary endorsements.
What we offer:
Total remuneration value of up to $132,567 p.a. is comprised of:
- An hourly rate between $43.68 - $58.59 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
