About the Role

We are seeking a dedicated Social Worker to join our multidisciplinary team. As a Social Worker, your responsibilities will include:

• Providing counselling and support to patients and their families/carers.
• Coordinating the timely provision of community services essential for safe discharge.
• Meeting with patients and carers throughout their hospital stay to address their needs and concerns.
• Developing accurate discharge plans and maintaining thorough documentation.
• Establishing and maintaining relationships with community services.
• Educating patients and carers on healthcare information.
• Facilitating and coordinating debrief sessions with the multidisciplinary team.

Requirements: To succeed in this role, you will need:

• A degree in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
• Demonstrated ability to manage a demanding workload and prioritize effectively.
• Strong interpersonal and communication skills, essential for working within a multidisciplinary team and with patients.
• Commitment to continuing education and professional development.
• Computer literacy and proficiency in documentation.
• A commitment to quality improvement and patient care.

Desirable Criteria:

• Demonstrated experience or expertise in hospital care.
• Flexibility and the ability to adapt to dynamic work hours.
• Experience or interest in group work.
