About the role

The Tierney House Residential Support Worker is responsible for delivering high-quality, compassionate care to residents, ensuring their physical, emotional, and social well-being, and supporting their journey out of homelessness.

  • Provide 24-hour care and supervision for residents, ensuring a safe and supportive environment for health and well-being.
  • Collaborate with the Homeless Health Service and other partners to enhance health outcomes and connect residents with vital services and resources.
  • Conduct comprehensive assessments and care planning for residents, coordinating referrals to other services as needed.
  • Maintain accurate documentation and electronic records, ensuring clear communication of care plans and progress.
  • Promote resident integration into the community and support their transition to mainstream healthcare and services.

About you
We are seeking a compassionate and dedicated individual who is committed to providing high-quality support to residents in need.

  • Experience supporting residents experiencing homelessness, ideally within a residential setting or Specialist Homeless Service.
  • Strong verbal and written communication skills, with the ability to engage effectively with residents, families, staff, and stakeholders.
  • Ability to work independently and as part of a collaborative team, demonstrating initiative and responsibility.
  • Proven experience in developing and implementing care plans to assist residents in their daily living activities.
  • Cultural competency and experience working with diverse populations, including those with co-occurring mental health, substance use, and physical health issues.
