About the role:
Full time, Monday to Friday (75 hours per fortnight) (Negotiable)
Choice of location – Port Augusta or Whyalla
The Indigenous Health Project Officer will review and provide guidance on Integrated Team Care (ITC) activity and deliver projects that contribute to the overarching aims of the ITC activity’s being to:
Contribute to closing the gap in life expectancy by improved access to culturally appropriate mainstream primary care services (including but not limited to general practice, allied health and specialists) for Aboriginal and Torres Strait Islander people.
Provide outreach support and program coordination to develop multi-program approaches and cross-sector linkages by promoting services to external stakeholders.
Increase awareness to maximise links between Aboriginal Health Services and mainstream services through the uptake of health assessments, care planning and follow up.
About You:
Qualifications and/or skills and experience working with Aboriginal and Torres Strait Islander People.
Experience working in primary health setting with a sound knowledge of general practice, primary health care and the social and emotional well being needs of Aboriginal and Torres Strait Islander peoples.
Exceptional demonstrated ability to build customer relationships, communicate ideas and information, planning, problem solving in relation to the objectives of the activity to both internal and external stakeholders
Demonstrated experience in delivering or coordinating culturally appropriate training and quality improvement activities
Experience in coordinating relevant education or health promotion events to the broader community, for example through community days, websites, conference presentations, at meetings and in reference groups for other projects.
