Affinity Disability Services provides NDIS registered services including Support Coordination, Psychosocial Recovery Coaching and Core supports to NDIS participants. We are located in Ipswich QLD, but service Brisbane, the Lockyer Valley Region and Brisbane Scenic Rim.

Your participants could be located within Brisbane and Logan areas.
Please note:
This role includes being able to deliver NDIS Support Coordination which involves supporting participants to work with their providers, the NDIS and other stakeholders to implement their NDIS plan. The role is not a counselling, life coaching, or a support work role.
Experience with working with the justice system (such as Probation and Parole, Corrections) is required as we are searching for a FORENSIC SUPPORT COORDINATOR.
Your role will include (but is not limited to):
Support participants with disabilities to manage complex challenges in their day to day lives.
Support participants to build capacity so that they can experience greater social and economic independence.
Support participants engaged with the Justice system - this may include liasing with Queensland Correction Services, Justice Planning team at NDIS and Prisons.
Assist NDIS participants in Support Coordination and implementation of all services in their funded plan.
Work collaboratively with participants, their families, carers and other services to ensure accurate use of their NDIS funds is implemented.
Maintain accurate and up-to-date case notes on service provision.
Establish and maintain effective links with the NDIA, other service providers and relevant stakeholders.
Engaging in case management including: home visits, stakeholder meetings, phone and email correspondence etc.
Qualifications & experience
Support Coordination: relevant degree qualification and/or equivalent industry knowledge and skills from experience in sector.
Blue Card, Yellow Card (workers screening) and suitable Police Check certificate; or willing to obtain.
Current Australian Drivers License.
