About the role

Holistic Care Nursing and Disability Services' is seeking a dedicated and compassionate Disability Support Worker to join our team in Western Sydney, NSW. As a casual Disability Support Worker, you will play a crucial role in providing individualized care and support to our clients, empowering them to live independently and achieve their personal goals.

What you'll be doing

Assisting clients with their daily living activities, such as personal care, meal preparation, and household tasks

Facilitating access to community services and resources to promote social inclusion and participation

Implementing tailored support plans to help clients achieve their personal, social, and recreational goals

Providing companionship, emotional support, and encouragement to clients

Maintaining accurate records and documentation to ensure high-quality service delivery

Collaborating with multidisciplinary teams to ensure the best possible outcomes for clients

What we're looking for

A Certificate III in Individual Support (Disability) or equivalent qualification. 2nd and 3rd year Nursing students welcome to apply.

Demonstrated experience in providing person-centered support to people with disabilities

Excellent communication and interpersonal skills, with the ability to build positive relationships with clients and their families

A caring, empathetic, and patient approach to supporting clients with diverse needs

The ability to work collaboratively in a team environment and follow instructions

A valid driver's license and access to a reliable and insured vehicle is mandatory
