Companion Health is a registered NDIS provider based in Perth, committed to delivering high-quality, client-centred care for individuals with diverse needs throughout the Perth metropolitan area. As we continue to expand, we are seeking an experienced, dedicated Service Coordinator to lead the delivery of high-quality, person-centred support across our supported accommodation homes.

This is a leadership role where you'll mentor frontline House team leaders and Support staff, ensure service compliance, manage stakeholder relationships, and contribute to the overall quality, safety, and wellbeing of the individuals we support.

Key Responsibilities:

Supervise day-to-day operations across multiple supported homes

Provide ongoing mentoring, guidance, and performance support to House team leaders and Support Workers

Ensure services align with individual care plans and NDIS guidelines

Monitor and manage NDIS funding, budgets, and participant outcomes

Conduct regular home visits, audits, and quality assurance checks

Liaise with families, health professionals, and other stakeholders

Oversee rosters, incident reports, documentation, and service compliance

Contribute to strategic planning, staff recruitment, training, and development

Create and maintain a culture of safety, professionalism, and client-first service

What You’ll Need:

Diploma of Community Services (or relevant qualification in client services or similar field)

Demonstrated experience in the disability or NDIS sector, with a strong understanding of person-centred practices and NDIS Quality & Safeguarding Framework

Proven leadership skills in team supervision, mentoring, and case management

Excellent communication, conflict resolution, and stakeholder engagement skills

Sound knowledge of rostering, budgeting, and compliance requirements

Proficiency in Microsoft Office, NDIS portals, and reporting systems

WA Driver’s Licence, NDIS Worker Screening Check, and completion of required NDIS modules

Flexibility to attend appointments or work outside standard hours when needed
