About Nothern Health:

Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north.

Services are provided through our five main campuses:

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Centre, and Kilmore District Hospital.

In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.

About Mental Health Division:

Northern Health is the third largest mental health service in Victoria Northern.

Within the Mental Health Division, we pride ourselves on providing high quality mental health services to people living in the northern regions of Melbourne, and work in a collaborative way with consumers and carers based on recovery principles.

We aim to provide strong clinical leadership to support all staff in the provision of professional treatment and care by our integrated community teams, inpatient, and residential services.

About the Role:

Are you experienced as an Occupational Therapist, Psychologist, Registered Psychiatric Nurse, or Social Worker? This may be an excellent opportunity for you!

In the role of Housing Pathways Coordinator you will provide leadership and recovery focused interventions that respond to the housing needs of our consumers.

You will be responsible for working in collaboration with consumers, community clinicians and families/carers, to support identification of appropriate housing options.

In this role you will provide consultation and support to staff within the Inpatient units.

You will have the responsiblity for the development of an organisational comprehensive understanding of the local and extended housing system and accommodation available including strategic work, policy development and housing stakeholder engagement.

The role will be required to work across various sites and the division catchment. This is role is a full time ongoing position (part time considered).

To be successful in this role you will have:

  • An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers.
  • An approved degree in Occupational Therapy and current registration with the Occupational Therapy Board of Australia/ AHPRA.
  • Must have experience working within or with the housing sector and an in-depth knowledge of the local and state housing service system.
  • Extensive relevant experience in a mental health or in the acute hospital setting and within the housing sector.
  • Knowledge of the Royal Commission into Victoria's Mental Health System Recommendation related to 25.3 - Social Housing.
  • Experience and well-developed skills working with consumers, individuals, families and groups, consultation and liaison with relevant housing services.
  • Relevant knowledge of and demonstrated skills in the principles and practice of working with people with a psychiatric disorder or disability, and their carers, families and dependent children.
