Join Our Team | Placement & Practice Support Officer

Location: Newcastle & Surrounds | Part-Time or Full-Time (Flexible)

We're currently on the lookout for a passionate, organised, and purpose-driven team member to join us in a pivotal role supporting our Individual Living Options (ILO), Short Term Accommodation (STA), and community-based placements.

About the Role:
This dynamic position plays a key part in supporting our operations teams to deliver high-quality, trauma-informed support to the people we walk alongside. No two days will be the same! From coordinating updates and supporting logistics to fostering meaningful collaboration across care teams — you'll be the glue that helps everything run smoothly.

Key Responsibilities:

Coordinate and write Placement Updates & Progress Reports

Oversee Medication Management tracking and documentation

Process Intake Paperwork and Placement Ordering

Complete Life Story Work in collaboration with participants

Manage STA logistics, communications, and shift allocations

Prepare and submit RP reporting and Panel Requests

Maintain and update Roster of Care documents

Conduct new client and team interviews

Assist with Incident Report follow-ups

Develop Safety Plans and Risk Assessments

Maintain Case Load Tracker & Audit Shift Notes

Liaise with providers to deliver participant updates

Monitor upcoming appointments & ensure team awareness

Confirm timesheets & ShiftCare accuracy weekly (by 10am Monday)

Schedule and minute internal team meetings

Manage property communications and logistics

Create visual aids and tools for participant routines

Support stakeholder engagement and collaboration

Maintain regular contact with clients to enhance engagement
