About Your Role

MLHD Chief Pharmacist - Aged Care, is responsible for the management and efficient performance of the Aged Care On-site Pharmacists (ACOP) team to improve medication management and safety for MLHD Aged Care residents.

The primary objective of the Chief Pharmacist, Aged Care is to optimise medication management and improve medicines-related outcomes for residents of MLHD Aged Care Facilities.

The pharmacist would work with other members of the multidisciplinary, managers as well as the older person (and their family/carers where appropriate), to ensure that pharmacotherapy is rational, safe, cost-effective, aligned with the person’s health care goals and preferences, and manageable without excessive treatment burden.

The position will be required to undertake a clinical and leadership role, including support to ACOP pharmacists.
