Clinical (RN) Case Manager – Full time

We build a team of nurses and support workers around our clients to ensure that they have consistency of care, and our employees have regular shifts with the same clients, so they build that relationship.

As a Case Manager you play a vital role in the management of our clients and their supports.

We are looking for a great Clinical Case Manager to join our Head Office team based in Southport. Our model works best with you being an excellent RN with solid nursing experience. As the care coordinator/case manager for our complex clients you provide clinical oversight along with mentoring and supporting of our community team of nurses and support staff.

Ideally you have experience with complex health conditions, and you are solid with undertaking comprehensive health assessments, experience with diabetes, catheter management, PEGs and wounds.

Customer service will be important to you, as its important to us! You will have strong relationship management skills and be adept at advocating for your clients whilst liaising with multidisciplinary teams.

The role is a mixture of being in the office and being out in the community visiting clients, training your support team, undertaking assessments and onboarding new clients. There are company vehicles available for use.

To be successful you will have:

· Qualified Registered Nurse with preferably 2-5 years experience

· Previous case management and/or supervisory experience is desirable, but not essential

· Ability to work collaboratively in our multidisciplinary team

· Highly developed verbal and written communication skills

· Confidence in managing your time effectively including the ability to work autonomously and part of a team

· IT savvy

· Blue card and NDIS Workers Screening Clearance or willingness to obtain

· Full, unrestricted AHPRA registration and legal working rights in Australia

· Current Australian drivers licence

· Remuneration is paid above the Nurses Award rate of pay

· Be available to participate in the On Call roster, once every 8-10 weeks (and paid a generous on call allowance). Don’t worry you don’t go on the On Call roster until you are fully settled into your role and are deemed confident!
