About the role

Tumut Regional Family Services Inc. is seeking a passionate Transitional Housing and Outreach Case Manager to join our team. As a part-time role based in Tumut, NSW, you will be responsible for providing critical support and case management to individuals and families facing housing insecurity and homelessness in the Snowy Valleys and Gundagai LGAs.

This is a part-time position with a maximum term contract until 30 June 2026 and further if future funding is made available. An additional contract may be offered.

What you'll be doing

• Delivering holistic case management to clients, including coordinating access to transitional housing, supporting clients to achieve sustainable long-term housing, and connecting them with essential support services
• Conducting comprehensive assessments to identify client needs and develop individualised support plans
• Advocating on behalf of clients to secure appropriate housing and community resources
• Maintaining detailed case notes and records in accordance with organisational policies and procedures
• Collaborating with a network of community partners and service providers to ensure seamless client support
• Participating in team meetings and professional development activities to continuously improve service delivery

What we're looking for

• Relevant tertiary qualification in community services, or a related field. Or currently working towards qualifications.
• Strong understanding of the complex issues facing individuals and families experiencing homelessness or housing insecurity
• Excellent communication and interpersonal skills, with the ability to engage sensitively with diverse client groups
• Demonstrated ability to work collaboratively within a team and across multiple stakeholders
• Proficient in case management software and data recording systems
• Current driver's licence and valid Working With Children Check.
• Must be available for on-call roster.
