Moorditj Koort Aboriginal Corporation (MKAC) is an Aboriginal community controlled, not-for-profit organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples living in Western Australia.

The Vision is to contribute to healthy and flourishing Aboriginal communities across WA by providing access to a holistic one-stop-shop for health and wellbeing support.

About the role:

The Housing Case manager is a collaborative role developed between Anglicare WA and Moorditj Koort Aboriginal Corporation (MKAC) to provide culturally safe and holistic case management support to individuals and families experience housing challenges. This role focuses on maintaining tenancies, preventing homelessness and addressing broader social, emotional and financial needs through integrated support.

Responsibilities and Duties:

Case Management

Conduct holistic assessments of clients’ housing, cultural, social, and financial needs.

Develop, implement, and monitor tailored case plans in collaboration with clients, ensuring cultural sensitivity and alignment with client goals.

Facilitate access to housing, tenancy support, and other relevant services, advocating on behalf of clients where required.

Provide guidance and support on tenancy rights and responsibilities, ensuring compliance with tenancy agreements.

Collaboration and Partnerships

Collaborate with external stakeholders, including landlords, property managers, and other service providers, to address barriers to housing stability.

Represent the program in community and interagency meetings.

Promote and strengthen partnerships between Anglicare WA and MKAC to enhance service delivery and community outcomes.

Cultural Safety and Community Engagement

Ensure services are culturally safe and inclusive, respecting the diverse needs of clients.

Engage with local Aboriginal communities to understand housing needs and co-design solutions.

Refer clients to appropriate services as needed

Program Administration

Maintain accurate and up-to-date client records in compliance with both organisations’ policies and confidentiality requirements.

Contribute to reporting requirements for both Anglicare WA and MKAC, ensuring key performance indicators (KPIs) are met.

Participates in Quality improvement initiatives and processes

Participate in regular supervision, team meetings and contribute to a collaborative and supportive workplace.

The successful candidate must have the following essential certifications.

Previous experience in a similar role.

WA Valid Driver’s license

Police Clearance no more than 3 months old

First Aid or willing to obtain.

COVID-19 Vaccination Certificate

Valid Working with Children's Check

Skills, Abilities and Knowledge

Relevant qualifications in social work, community services, or a related field and/or extensive experience.

Demonstrated experience in case management, preferably in housing or homelessness services.

Strong understanding of Aboriginal and Torres Strait Islander cultures, histories, and contemporary issues, particularly in relation to housing and wellbeing.

Proven ability to work collaboratively across organisations and with diverse stakeholders.

Excellent communication and interpersonal skills, with a client-focused approach.

Strong analytical, problem-solving and advocacy skills.
