This Position requires an Individual who has:

Experience providing NDIS Support Coordination/Recovery Coaching Services or Similar Role

Experience with Mental Health and Complex Care

Experience with Justice Background (Preferred)

Excellent Time Management and Administration with the ability to maintain Accurate, Professionally appropriate and Current Records and Written Reports.

A Solid understanding of the NDIS and its Importance to People with a Disability and their Support Networks.

Demonstrated experience with Facilitating Multi-Disciplinary Coordinated Planning and Develop Collaborative Relationships and Networks between Service Providers.

Maintaining an Up-To-Date knowledge of the NDIS Systems and Practices;

Engaging with NDIA Staff and LAC’s;

Liaising with the Participant, Family or Representative on Supporting the Participant with Achieving their Goals

Preparing for Plan Reviews and Attending with the Participant, Family or Representative

Qualifications & Experience:

Preferably 1.5 Years of Minimum Experience in this role or similar

Experience with Engaging with Stakeholders and Ability to Adapt

Preferably Qualifications with in the Community Services, Mental Health and Disability/and or Similar.
