The Opportunity

As a Support Coordinator with AQA, you will support people with disabilities to navigate service systems, explore support options, and achieve their goals.

Your work will reflect AQA’s values of integrity, trust, respect and excellence, and will be guided by our Service Partnership model - an approach that centres collaboration, empowerment, and transparency.

You’ll work within a supportive team, drawing on the knowledge and experience of peers and professionals alike. Lived experience is central to the work we do, and you’ll be part of a community that builds capacity through partnership and shared learning.

What You'll Be Doing

Provide Support Coordination and Case Management across NDIS and other funding bodies.

Empower participants to access a range of formal, informal, and community-based supports.

Promote choice and control through innovative, person-centred service delivery.

Implement, monitor, and review individual support plans.

Identify and troubleshoot barriers to service delivery.

Build participant capacity to manage and maximise their NDIS plans.

Maintain accurate case notes, service and billing records, and progress reports.

Deliver and support information sessions, pre-planning workshops, and 1:1 consultations.

Collaborate with participants, families, and providers to achieve great outcomes.

Contribute to a positive team culture, learning from and supporting one another.

This role offers a combination of office-based and remote work.

What’s in Your Toolkit?

A Certificate IV, Diploma, or Degree in Disability, Mental Health, Community Services, or a related field.

At least 2 years experience in the disability, mental health, or community services sector.

A solid understanding of the NDIS, its framework, and how it can support people to live independently.

Strong communication and interpersonal skills, with the ability to build trust and rapport.

A collaborative, solution-focused approach.

Excellent written documentation and time management skills.

Proficiency in Microsoft Office, Google Suite, and CRM systems.

A genuine commitment to supporting others to thrive.

Essential Requirements

NDIS Worker Screening Check

Working with Children Check

Current Australian driver’s licence

Evidence of COVID-19 vaccination or valid exemption
