About the Role
As a Service Coordinator, you will play a key role in overseeing participant intake, planning, and support transitions. You'll work closely with participants, families, and multidisciplinary teams to ensure high standards of service while fostering a positive and collaborative workplace culture.

Key Responsibilities

Lead intake, planning, and participant transition processes

Educate and support staff to ensure high-quality, person-centred care

Build strong partnerships with internal teams and external providers

Monitor staff performance and ensure compliance with NDIS standards

Support participants in preparing for reviews and reporting outcomes

Coordinate and manage incident reporting and investigation processes

Qualifications & Experience

Strong understanding of NDIS regulations and compliance

Excellent interpersonal and communication skills

High-level organisational and report-writing abilities

Demonstrated experience in person-centred service coordination

Flexible, resourceful, and innovative approach to problem-solving
