About the Role & Client

We are proud to partner with an Aboriginal Community Controlled Health Service in South Hedland, WA, delivering integrated, person-centred care across the Pilbara region. This organisation ensures clients' journeys through the health system meet their medical, health, social, and cultural needs. Their vision is for Aboriginal and Torres Strait Islander people in the Pilbara to have the opportunity to live a healthy life, supported by culturally appropriate, holistic healthcare, social and emotional wellbeing services, and education.

As the Social Worker, you will support the Australian Family Partnership Program (AFPP) team and broader service needs, delivering social and emotional support, advocacy, and facilitating access to essential community resources for clients and families.

About the Service

The AFPP focuses on maternal and child wellbeing, empowering families through home visits, health education, and goal-setting. The program supports families affected by trauma, domestic violence, mental illness, or substance use, promoting culturally safe, strength-based care in partnership with other health professionals and community stakeholders.

Key Responsibilities

Counselling & Case Management:

  • Provide emotional support and mental health counselling to individuals, families, and groups
  • Conduct psychosocial assessments and refer clients to appropriate services
  • Deliver crisis support and ongoing case management

Client Advocacy & Community Support:

  • Link clients with relevant Hedland-based and regional support services
  • Advocate on behalf of clients and families to ensure access to care
  • Maintain up-to-date and accurate client documentation and reports

Collaboration & Engagement:

  • Work collaboratively with multidisciplinary teams to deliver holistic care
  • Increase Aboriginal engagement in health services
  • Build strong relationships with stakeholders and community partners
  • Travel to remote communities as needed to extend outreach

Skills & Requirements

Essential:

  • Degree in Social Work and relevant registration
  • Experience working with clients impacted by child abuse, family violence, mental health issues, and parental substance misuse
  • Strong understanding of Aboriginal primary health care and the social determinants of health
  • High-level stakeholder engagement and communication skills
  • Ability to work independently and manage competing demands
  • Competence in using client management systems (e.g., Communicare)
  • Current Driver's Licence, Working with Children Check (WWCC), and National Police Check
  • Commitment to ongoing professional development

Desirable:

  • Experience working in remote health settings, particularly with mothers and children
  • Knowledge of social justice and systemic issues affecting Indigenous communities
